Zusammenfassung

Das Placebo ist der Freund des Patienten und des behandelnden Arztes und der Feind des Wissenschaftlers, der Gewissheit über die Wirksamkeit neuer Medikamente haben will. Ohne die unterstützende Wirkung des Placeboeffekts wären aber viele Medikamente weniger wirksam. Bei weiterem Fortschritt der Neurowissenschaften kann man hoffen, dass durch weitere Abklärung des Placeboeffekts gezielt jene Mechanismen, über die Placebo seine Wirkung entfaltet, pharmakologisch beeinflusst werden können. Eine Kategorie von Medikamenten mit akzeptabler Wirkung und ohne Nebenwirkungen könnte entstehen. Bis dahin müssen wir uns damit begnügen, die jetzt schon bekannten Methoden, den Placeboeffekt auszulösen, maximal zu nutzen.

Abstract

Placebos are pills, drops or injections which do not contain drugs with biological or pharmacological activity. Nevertherless, under certain circumstances placebos may exert a beneficial (“placebo effect”), but sometimes also a negative (nocebo) clinical effect. Whether or not a placebo effect occurs after application of a placebo depends on a variety of factors which include the personality of the “healer”, the type of placebo, the expectations of the patient, the type of symptoms or of the disease and many other factors. The potency of the placebo effect can be best demonstrated in well conducted double blind, placebo controlled clinical studies. The strongest placebo effects have been seen in pain-associated disorders and in depression, but also in some immunological disorders. The perceived placebo effect consists of several components such as spontaneous improvement of symptoms, statistical factors (“regression of the mean”), conditioning and the “true” placebo effect. Recent data indicate that the true placebo effect is mediated through activation of certain receptors in the brain. The use of placebo controls in clinical trials for the evaluation of new drugs is important, but in some instances placebo controls may be problematic from the ethical point of view.
