Zusammenfassung

Appetitverlust, Anorexie und Kachexie sind multifaktoriell bedingte Symptome, die in palliativen Situationen einer fortgeschrittenen Erkrankung sehr häufig auftreten. Die zentrale Ursache für den Verlust des Appetits bei schwerkranken und alten Menschen ist eine katabole Stoffwechsellage. Dieser auf Abbau ausgerichtete Prozess ist auch durch hochkalorische Nahrung nicht umkehrbar. Während in der kurativen Phase einer Erkrankung eine Wiedererlangung der Kraft durchaus ein realistisches Ziel sein kann, sind die Stabilisierung des Körpergewichts und eine Zunahme an Kraft in palliativen Situationen aufgrund der verschiedenen Abbauprozesse nicht mehr möglich. Im Zentrum der Bemühungen liegt die Erhaltung des Appetits, die Bewahrung der Freude am Essen, unabhängig von Menge und Nährwert der aufgenommenen Speisen.

Abstract

Loss of appetite, anorexia, and cachexia are multifactorial symptoms that are common in the palliative treatment of advanced disease. The central cause of a loss of appetite in critically ill and elderly people is a catabolic metabolism. This degradation-focused process is also irreversible with high-calorie foods. While regaining strength in the curative phase of a disease may be a realistic goal, stabilizing body weight and increasing strength in palliative situations is no longer possible due to the various stages of degradation. At the centre of such efforts is the preservation of appetite and the preservation of the enjoyment of food, regardless of the quantity and nutritional value of the food consumed.
